Include the following information:
- Name
- Shipping address
- Part number of the damaged item
- Electric fireplaces: include the mantel lot # which can be found on the front of the instruction manual or on the back of the damaged part.
- Outdoor fireplaces: include the model number which can be found on the front of the instruction manual or on the control panel.
